Chapter 316 Communication with the Chief Resident
Chapter 316 Communication with the Chief Resident
"Xiao Ji, how did you figure that out?"
Upon returning, Deng Chaohong asked with a belly full of suspicion.
"Teacher Deng, I didn't see it," Ji Xiang said casually. "I just noticed that the patient's feet were a bit off in color. As for Budd-Chiari syndrome, I just remembered what I read in the book and made it up. Please don't laugh at me."
"Is that so?" Deng Chaohong looked at Ji Xiang seriously, sizing him up as if it were their first meeting.
The patient's case was rather unusual, as Budd-Chiari syndrome rarely presents with symptoms of orthopedic spinal stenosis.
I remember a patient in the proctology department whose hemorrhoids kept recurring after being removed, and finally, after a full-body examination, it was diagnosed as Budd-Chiari syndrome.
Such situations are extremely troublesome, as even a clear diagnosis is difficult to come up with. The patient's symptoms are atypical, which would be a challenge for anyone.
If Ji Xiang hadn't been here today, I probably would have made a misdiagnosis.
Recalling his earnest advice to Ji Xiang, Deng Chaohong blushed even more.
"Let's go eat," Deng Chaohong said.
"Teacher Deng, how many patients do you usually see in your clinic each day?" Ji Xiang didn't mention Bugatti Syndrome, and chatted with Deng Chaohong about everyday things while packing his things.
Deng Chaohong had always been very interested in Ji Xiang, and he knew that this young man had unlimited potential.
After returning to the hospital after lunch, Deng Chaohong spent the afternoon in the outpatient clinic. When he was about to leave work, he said to Ji Xiang, "Dr. Ji, try to talk to General Manager Xu more when you have time. Our orthopedic emergency surgeries are all performed by the resident general manager. It won't hurt to get to know him better and perform more surgeries."
"Thank you, Teacher Deng."
Ji Xiang also felt that he should spend more time in clinical settings. Although there are operating rooms, that's not the whole picture.
The operating room is more like a training ground; you have to put it to the test to see what it's really like.
He returned to the ward without changing his clothes, and instead went to find Xu Leyu, the chief resident of orthopedics.
"Mr. Xu, how many emergency cases do you see in your orthopedics department at night?"
As Ji Xiang walked to the office door, he heard Lu Kai chatting with General Manager Xu Leyu.
Lu Kai's communication skills are truly outstanding. He was like that in the urology department, and after coming to the orthopedics department, he quickly became very close with the head of the ward.
His tone was familiar and natural, this chubby boy had a natural affability. Moreover, he was thick-skinned and outgoing, qualities that Ji Xiang lacked.
Ji Xiang smiled and walked into the office.
"It's alright, not too much, not too little. If I really want to have surgery, I can work non-stop." Xu Leyu glanced at Ji Xiang and replied simply.
Xu Leyu has fair skin and a scholarly air about him, making him look like a popular young actor.
"Whether it's urgent or not isn't important, the main thing is to avoid any accidents," Xu Leyu, the general manager, said to Lu Kai. "You heard what the director said this morning, right?"
"I heard about it. I went to inquire about it today. I guess the head of the gastroenterology department was busy with emergency cases that day and didn't say a few words, so the patient's family misunderstood," Lu Kai said.
"A misunderstanding, it's a minor matter." President Xu smiled wickedly, looking at Lu Kai. "Do you know what the most important skill for a chief resident is?"
"Surgery? Diagnosis?" Lu Kai replied.
"No, the most important skill is face reading."
"!!!" Lu Kai was stunned.
Ji Xiang also found it interesting and sat down to listen carefully to what General Manager Xu had to say.
"I'm afraid you won't understand if I just say it, so let me give you an example," said General Manager Xu. "A few days ago, there was a complaint in the glandular department. It was quite frightening. The patient's mother insisted that the surgeon had pricked her son's carotid artery with a needle, almost killing him."
"Uh, that can't be right, this sounds really weird..." Lu Kai's voice trailed off as he spoke.
"The rumors were really exaggerated at the time. I went to take a look, and the patient's family even took pictures. When I saw it, I realized that it wasn't the carotid artery at all; it was probably a scar from the removal of the drainage tube after the thyroidectomy."
"Wow, you can complain about something like this? And you're making it sound so outrageous? Have social media platforms gone crazy with these kinds of claims?" Lu Kai exclaimed in surprise.
"This is clearly a case of the patient's family not understanding and scaring themselves. I don't think they have any ulterior motives. If they really did, what medical record in the hospital wouldn't have some flaws? If they were just writing medical records all day, would they even be able to perform surgeries?"
"Mr. Xu, you're right," Lu Kai chimed in, playing the straight man.
"That's why face reading is so important." Mr. Xu lay lazily in his chair, looking as if he was about to fall asleep. "Especially when we're treating emergency patients, sometimes we have no choice but to worry about how troublesome the patients and their families are, and whether they'll make up complaints about us out of thin air. We need to be aware of that."
"Mr. Xu, could you tell me about some examples you've encountered?" Lu Kai asked with a smile.
Because they weren't familiar with each other, he spoke to General Manager Xu rather politely, addressing him as "you" (formal). In the urology department, Lu Kai and General Manager Wu spoke like old friends, and the chubby guy knew this all too well.
"There's not much to say," Mr. Xu smiled. "We have emergency cases every day; you'll see when you see it. This is something that won't be written in textbooks, but it's one of the most important parts of clinical practice."
"Most important?"
"You may not be good at treating patients, you may not be good at performing surgery, after all, there are senior doctors around," said General Manager Xu. "If there's anything, consult with them and report to them more often. Our Second Affiliated Hospital is different from other hospitals. Senior doctors... for example, the second-tier professor on duty today, if I ask him to perform surgery in the middle of the night, he won't scold me for being unreasonable."
"Uh, is that so?! Shouldn't second-tier doctors be treating difficult cases?"
"Of course," said Mr. Xu. "Being a doctor is a profession. Treating and saving lives is the result, not the goal. Don't let yourself be brainwashed before you even start working, indulging in a sense of superiority. Just talking about raising your status without mentioning money is just being a scoundrel."
"..."
"..."
Ji Xiang and Lu Kai didn't know what to say.
"Alright, let's order food. What would you like to eat?" Mr. Xu asked.
"Eat meat!" Lu Kai said with a grin, showing no politeness.
"And you, Dr. Ji?" Mr. Xu asked Ji Xiang.
No sooner had the words left her lips than a cry of pain came from the elevator entrance, followed by a faint, delicate scolding.
Mr. Xu's lips moved, his face showing displeasure.
"I've been having bad luck lately. Every time I mention eating, I end up in the emergency room," Mr. Xu said with a sullen expression. "Probably the emergency room, sigh."
As he spoke, he stood up and turned his head to look outside the door.
"Doctor, the emergency room sent us up," a young man said cautiously.
"Where are the X-rays? Let me see." Seeing the patient behind the man, supporting his left arm with his right hand, and his left shoulder slumping heavily as if weighed down by a weight, Mr. Xu knew that nothing was wrong and asked for the X-rays first.
"Here, here." The man immediately handed over the film.
Mr. Xu took the film, glanced at the female patient, and without asking her about her condition, inserted the film into the viewer.
The left clavicle is fractured, and the break is quite severe; even an ordinary person who cannot read X-rays can see it at a glance.
"How did you get injured?"
"Car accident."
