Great doctors start with pediatrics
Chapter 2 Misdiagnosis
As soon as the words were spoken, the consultation room fell silent.
Zuo Zhenxing's face suddenly darkened, his gaze sharp as he stared intently at Luo Lang.
The old lady also stopped, holding the boy, and looked at Laurent warily.
Laurent took a deep breath.
That was rash.
Resident trainees are at the very bottom of the hospital hierarchy.
Even in a regular doctor's office, interrupting a consultation or questioning a diagnosis is tantamount to suicide for a resident.
At best, you'll be severely reprimanded; at worst, you'll be made to suffer in the hospital, forced to voluntarily terminate your residency training.
If he had money, Laurent, who was a lawyer in his previous life, would certainly not have chosen to become a doctor.
But as Laurent watched the increasingly bright red light above the boy's head, his heart raced.
He could die! If it's missed, the boy might die!
He knew what the consequences would be.
But he didn't know why his body started moving involuntarily at that moment.
He's made a move, so we'll continue regardless of the outcome!
The boy could die if he left this clinic and hospital.
This is the only place that can save him.
Laurent remained silent.
With a clatter, the books beside him fell to the ground.
Laurent took three steps at a time and stood up to block the door.
If you gave him a gun, he could point it at everyone present right now and force the hospital to examine the child.
Zuo Zhenxing grabbed the table and quickly stood up, demanding, "Laurent, what are you doing?"
"Teacher Zuo, he can't go out!"
Laurent's palms were sweaty. He was a lawyer in his previous life, so he naturally knew the consequences of his current actions.
If something really happens to the patient, he might even become the first person to be sued.
He doesn't have any money!
But Laurent, suppressing his unease, said, "This child doesn't have a simple scratch."
"Then what do you think it is? Tell me!" Zuo Zhenxing shouted sternly.
Laurent hurriedly threw down his notebook, but didn't leave the doorway.
"What are you recording? What are you trying to say?"
Zuo Zhenxing's face scrunched up; he had never seen such a flawed notebook before.
He thought he could glean Laurent's thought process from the notes, but Laurent hadn't even done the basic note-taking.
Upon hearing this, the old woman realized what was going on and angrily said, "What's wrong with your hospital? One minute you say it's nothing, the next you say there's something wrong! Are you playing the good cop/bad cop for this old woman?! Do you just want my money?!"
An elderly woman with a child is like a mother cow protecting her calf, naturally wary of doctors.
"Laurent!"
Upon hearing the family's questions, Zuo Zhenxing quickly silenced the old woman, angrily shouting, "Get out of my way!"
"As a resident, you disregarded outpatient regulations and prevented patients from leaving the hospital. Do you know what kind of behavior this is? I'm calling the residency department now to terminate your residency training. You will never be able to enter clinical practice again in your lifetime."
As the deputy director of pediatric surgery, he certainly couldn't allow patients' families to have any complaints about the hospital.
But Laurent was caught in the middle, caught between the reprimands of authoritative experts and the questioning of his family, with no way to move forward or backward.
He had no medical experience and could not provide any evidence.
No one will trust results obtained through supernatural observation.
Moreover, Zuo Zhenxing had already completed the physical examination, which was in accordance with the procedures, and it could even be said that he had done his best.
But none of this prevents the light above the boy's head from turning deep red.
Seeing that Zuo Zhenxing couldn't persuade Laurent with just two sentences, the old lady trembled with anger. Holding the child, she shouted, "Are you crazy? Get out of the way, or I'll call the police!"
The boy was also frightened by the tense atmosphere, clutching his grandmother's sleeve tightly, his shoulders twitching slightly.
Her little mouth was slightly open, as if she wanted to cry, but for some reason she didn't.
Laurent broke out in a cold sweat, his mind racing.
Of course he could step aside; he was just a resident trainee, and the boy's condition and its consequences were none of his concern.
But as a lawyer, as a medical student, and as an adult—he couldn't back down when facing a child.
Staring intently at the red light, Laurent desperately tried to recall the "Pediatrics" and "Diagnostics" books he had been reading over the past two days.
Knowledge in my mind is like a tangled mess.
"Fell, landed on both knees...no external wounds..."
"Are you going to let me pass or not?" Zuo Zhenxing ignored Laurent's mutterings.
The current situation is undoubtedly serious for the hospital.
Zuo Zhenxing's face was ashen, his square face resembling the reverse side of a mahjong tile. He had already pulled his phone out of his pocket: "If you don't move aside, I'll call security and the residency training department right now!"
"Teacher, give me one more minute."
He is a lawyer from the north, and surprise evidence raids are as commonplace for him as drinking plain water. He can talk eloquently about evidence he has never seen before.
But faced with knowledge he didn't understand, Laurent couldn't make sense of it and finally gave the only conclusion he could be sure of: "I think the child may... have internal injuries."
"Internal injury? What's your basis for this? What signs were found on the physical examination? What are the diagnostic criteria?" Zuo Zhenxing pressed closer, walking up to the old woman and the boy, protecting them completely before questioning them:
"I know you young people want to prove yourselves and get opportunities, but you can't just rely on wishful thinking. Leave now, and I'll give you a chance."
Laurent did not respond.
Only a suitable reason can persuade someone.
He can't very well tell people that he diagnosed it as an internal injury simply because there were no external injuries.
To leave or not to leave?
There was no time to choose.
Laurent glanced at it out of the corner of his eye, pinpointed the location, bent down quickly to pick up the "Diagnostics" book he had dropped as he stood up, and flipped through the pages rapidly: "Fall, internal injury..."
"If it's an internal injury, we can start by looking at the abdomen." Suddenly, Fang Qingxue, who was usually silent, spoke up.
Laurent looked up at Fang Qingxue gratefully, then quickly flipped through the pages. His eyes suddenly lit up, and he said urgently, "Yes! Right here, the Kehr sign, a sign of a ruptured spleen! After bleeding in the abdominal cavity, the blood will stimulate the left diaphragm, and the pain will travel through the nerves to the left shoulder, forming referred pain."
"Teacher, could you examine the child's abdomen? Or could you massage his left shoulder?"
I just noticed that the child has twitching in his left shoulder; there are signs of this.
Zuo Zhenxing frowned and asked, "Are you leaving after I finish the re-examination?"
Zuo Zhenxing needs a definite guarantee.
Laurent was, of course, unsure if this was correct, but he figured he might as well try anything; the more tests the child underwent, the better the chances: "Yes."
Upon hearing this, Zuo Zhenxing turned around and said to the old lady, "The physical examination is free. Can I have your grandson checked? It's good for the child."
"You see my student too..."
Without needing Zuo Zhenxing to say anything more, the old lady quickly nodded in agreement.
However, he kept muttering, "I'm definitely going to file a complaint against your hospital later."
But Zuo Zhenxing didn't mind; he simply asked the old lady to lay the boy flat on the examination bed.
Lying in bed, the boy's face seemed noticeably paler than before.
"Don't be afraid, Uncle is just giving you a check-up."
Zuo Zhenxing stepped forward and gently pressed on the boy's upper left abdomen.
"Ouch, it hurts! Grandma!"
The child, who had been quiet and well-behaved during the physical examination, suddenly trembled violently and let out a sharp cry.
The old lady panicked instantly and tried to protect her grandson by stepping forward, but Fang Qingxue stopped her.
She wasn't some half-baked expert like Laurent; with the condition unclear, she absolutely couldn't let anyone touch the child even once.
Zuo Zhenxing frowned, took a deep breath, and asked the old lady with a serious expression, "Where else did your grandson get hurt? Tell me the truth! Was it only his knee?"
"No, that's all!"
Hearing Zuo Zhenxing's words, the old lady stammered, "It seems... ah! She bumped into the sofa when she fell... but I don't see anything wrong with her!"
Upon hearing this, Zuo Zhenxing didn't say much. He immediately took out his stethoscope and pressed on the left side of the boy's abdomen.
The boy cried out in pain again.
Zuo Zhenxing hurriedly grabbed the phone on the table: "Pediatric Surgery Clinic, Zuo Zhenxing, four-year-old child, highly suspected of delayed splenic rupture! Prepare FAST (bedside ultrasound), room 15, bring him here now! Open the green channel, urgently perform abdominal CT and blood routine tests."
"Xiaoxue, open the door to receive FAST."
Fang Qingxue acted immediately.
She was more familiar with hospital procedures than Laurent was.
"Laurent...you take care of the old lady."
The old woman's face turned deathly pale: "My grandson, has something happened to him?"
"Don't worry, nothing will happen."
