This miracle doctor just wants to be fired!
Chapter 4 Busy Departures
The ambulance rushed to the construction site on the outskirts of the city with its siren blaring.
Zhou Cheng sat in the passenger seat of the ambulance, while Nurse Zhao sat in the back.
Although he was thinking of just getting by, when it came time to see patients, his professional instincts made him involuntarily tense up.
Upon arriving at the construction site, in the dusty open space, a worker in blue overalls lay face up next to a pile of steel bars.
His left leg was bent outward at an odd angle, and his trouser leg was soaked in blood.
He gritted his teeth, cold sweat streaming down his face.
A group of anxious coworkers surrounded him; some wanted to help him up but dared not move.
Zhou Cheng immediately jumped out of the car, knelt down next to the patient, and immediately reached out to support the back of the patient's neck: "Don't panic, I am an emergency room doctor. Now listen to my instructions, don't move, I will examine you."
He first opened the patient's eyelids and observed the pupil's light reflex.
Both pupils were equal in size and round, with a diameter of about 3 mm, and showed brisk reflexes, ruling out an emergency situation of traumatic brain injury.
Next, he asked the accompanying nurse, Zhao Wenqi, to measure his blood pressure and heart rate; his vital signs were temporarily stable.
Then, Zhou Cheng gently lifted the patient's trouser leg.
The patient's left lower leg was swollen and deformed in the middle section. When the middle section of the tibia was pressed, the patient groaned in pain.
Zhou Cheng immediately stopped and instead touched the dorsalis pedis artery: "Can you feel the numbness in your foot? Try moving your toes."
The patient slowly moved his toes, and the dorsalis pedis artery pulse was clearly visible.
"Fortunately, no blood vessels or nerves were damaged."
Zhou Cheng felt a little more at ease, then turned to Zhao Wenqi and called out, "Bring the splint and elastic bandages!"
Zhao Wenqi worked quickly and handed over all the items in no time.
After taking the splint, Zhou Cheng carefully straightened the patient's left leg, placed the molded splint from below the knee to the ankle, and wrapped the upper and lower ends of the fracture with bandages three times each for fixation.
The tightness should be controlled to allow one finger to be inserted, avoiding compression of blood vessels.
"Now help him turn over, lift him with a spinal board, and be careful to keep his torso straight and avoid putting pressure on his lower back."
While directing his coworkers to assist, Zhou Cheng used saline solution to rinse the wound on the patient's leg and applied sterile gauze to stop the bleeding.
"Don't worry, it's a closed tibia and fibula fracture. The major arteries weren't damaged. We can get to the hospital within 20 minutes. Once we arrive, we can arrange examinations, and the subsequent surgery will be sufficient for repositioning."
Zhou Cheng's calm voice greatly eased everyone's tension.
Then, he and his fellow workers carefully lifted the patient onto the ambulance.
……
This was the first patient Zhou Cheng treated, and he was very thorough throughout the entire process.
Even if he wanted to be fired, he couldn't turn a blind eye to the suffering of his patients; it was a professional ethic that had been ingrained in him since he graduated from medical school.
Every examination action and every comforting word carries the muscle memory honed through years of clinical training.
After they took the worker with the broken bone to the hospital emergency room and handed the patient over to the on-duty doctor, before he could even take a sip of water, the emergency phone rang again.
"Dr. Zhou, a 72-year-old man in the Chengxi community suddenly experienced chest pain." Zhao Wenqi hung up the phone and sighed deeply. "Today has really been an unlucky day; I haven't been able to get any rest at all."
Zhou Cheng also shook his head slightly.
That's how it is with ambulances, especially since their city's People's Hospital is the leading hospital in the city, and most patients are sent here.
……
The ambulance arrived at the community quickly.
The building is an old building without an elevator, and the patient is still on the eighth floor.
Zhou Cheng and Zhao Wenqi, carrying the first aid kit and stretcher, climbed up to the eighth floor, panting heavily.
Inside the room, the old man sat back on the sofa, his hands tightly clutching his chest, his face pale and his breathing rapid.
The elderly man's family member, a middle-aged woman, stood by helplessly wiping away tears.
Zhou Cheng quickly stepped forward and first loosened the old man's collar to keep his airway clear.
Then, while instructing Zhao Wenqi to measure vital signs, he attached the electrocardiograph electrodes to the old man's chest, wrists, and ankles.
Waveforms quickly appeared on the electrocardiogram screen.
Zhou Cheng stared at the screen, his brows furrowed slightly.
On the screen, the ST segment of leads II, III, and aVF of the electrocardiogram shows mild ST segment depression and flattened T waves.
There was no ST-segment elevation typical of myocardial infarction.
At this moment, Zhao Wenqi said, "Dr. Zhou, the patient's blood pressure is 135/85 mmHg and heart rate is 92 beats per minute."
"Sir, does your chest still hurt? Is it a squeezing pain or a stabbing pain?"
Zhou Cheng leaned close to the old man's ear and asked loudly.
The old man shook his head weakly: "It feels suffocating, like a stone is pressing down on me."
"It sounds like unstable angina." Zhou Cheng took out nitroglycerin tablets from the first-aid kit and had the old man put them under his tongue. "Keep them in your mouth and don't swallow. If it doesn't get better in five minutes, we'll give him another one."
At the same time, Zhou Cheng instructed Zhao Wenqi to establish an intravenous access and infuse saline solution to maintain circulation.
Five minutes later, the old man's breathing gradually stabilized, and he was able to speak in complete sentences.
"Much better, I don't feel stuffy anymore." A sliver of color began to return to the old man's pale face.
The elderly person's family members standing nearby were also overjoyed and wept, repeatedly thanking Zhou Cheng and Zhao Wenqi.
Zhou Cheng had another electrocardiogram, and the ST segment depression had significantly decreased.
"Although the symptoms have lessened, we still need to go to the hospital for further examination," Zhou Cheng told the patient's family. "Unstable angina can turn into a myocardial infarction at any time, so we still need to be vigilant."
After the emergency treatment, the patient's daughter has complete faith in Zhou Cheng.
She nodded repeatedly: "Go to the hospital, you must go to the hospital."
Subsequently, with the help of the elderly man's family, Zhou Cheng and Zhao Wenqi carried him downstairs on a stretcher.
……
Inside the ambulance, Zhou Cheng and Zhao Wenqi both breathed a sigh of relief.
Although both of them were exhausted from running up and down the stairs, the patients were now safe.
In the ambulance, Zhou Cheng contacted the on-duty doctor in the hospital's emergency department and explained the elderly man's condition in advance.
Next, Zhou Cheng went to pick up several more patients.
After running around all morning, Zhou Cheng's white coat was covered in dust and bloodstains.
The ambulance driver still had time to eat something and drink some water.
However, he and Zhao Wenqi didn't even have time to eat biscuits; they only drank a few sips of mineral water during the ambulance's journey.
……
At noon, Zhou Cheng and Zhao Wenqi were getting ready to go for lunch.
Unexpectedly, the emergency call rang again.
This time it was in a very high-end residential area.
When Zhou Cheng and Zhao Wenqi arrived, a woman who looked to be about forty years old was sitting on a wooden chair in the living room, leisurely drinking tea.
"Excuse me, did you call the ambulance?" Zhou Cheng asked, frowning.
The woman nodded slowly, then clutched her chest with her right hand, frowning as she said, "I was so panicked just now, I couldn't breathe, and I felt like I was going to die any second. But I feel a little better now."
Although there seemed to be no problem, Zhou Cheng still stepped forward to conduct a physical examination, measure vital signs, and perform an electrocardiogram as usual.
The blood pressure results came back quickly: 120/75 mmHg, which is within the normal range.
Next, Zhou Cheng had Zhao Wenqi perform an electrocardiogram on the woman.
"The electrocardiogram is normal, it's sinus rhythm, and the heart rate isn't fast," Zhou Cheng continued, listening to the heart and lungs with a stethoscope.
The heart sounds were strong and there were no murmurs. The breath sounds in the lungs were clear and there were no moist rales, ruling out the possibility of heart failure and lung infection.
He then took out a blood glucose meter and had the woman draw blood from her fingertip.
Blood sugar was 5.8 mmol/L, which is also fine.
"Doctor, do I have heart disease? I always feel tightness in my chest and can't breathe properly. It's really uncomfortable. I almost fainted just now." As she spoke, the woman pulled out a thick stack of medical records from the TV cabinet drawer. There were more than a dozen of them.
"Look, I've been to the hospital six times in the last six months. Each time it was because of heart palpitations. I've had electrocardiograms, echocardiograms, and even coronary angiography, but they still couldn't find anything wrong."
"I feel better after staying in the hospital for a few days and getting some IV nutrition, but I feel unwell again as soon as I get home."
Zhou Cheng flipped through her medical records; her most recent hospitalization was last month.
Her discharge diagnosis stated "neurosis (somatization disorder)".
Having seen this, Zhou Cheng roughly understood the situation.
This is likely a typical case of physical discomfort caused by psychological factors.
Women may be in a state of anxiety for a long time, and the emotional stress may be transformed into physical symptoms.
However, Zhou Cheng didn't point it out directly. Instead, he patiently said, "Look, these tests all show that there's nothing wrong with your heart. It's probably because you've been too stressed and haven't been getting enough rest lately."
"If you're worried, you can go to the hospital at a later date for a Holter monitor test and an anxiety scale assessment. If hospitalization is required, you can go through the admission procedures and have a psychiatrist come for a consultation, okay?"
The woman shook her head: "No, I need to go to the hospital now! I won't feel at ease if I don't go. What if I have a relapse at home and no one is there to take care of me?"
But just then, the emergency center called again.
