Chapter 50 You Demonstrate
Chapter 50 You Demonstrate
Before their internships, every medical student will learn basic medical skills, including but not limited to the use of equipment, familiarity with medical devices, the four diagnostic methods of traditional Chinese medicine (inspection, auscultation, inquiry, and palpation), suturing techniques, and first aid methods.
However, the proportion of exploration and understanding far exceeds the actual operation.
Under the teacher's supervision, every student has the opportunity to practice. It is impossible to expect to master the skills, because the most important thing in the first two years of the six-year academic career is to solidify the theoretical foundation.
The middle two years were spent honing basic skills through departmental rotations, and the following two years were dedicated to becoming a qualified medical professional, preparing for the medical licensing exam. However, this did not mean abandoning theoretical foundations; on the contrary, the focus became increasingly diversified, with each additional area of study becoming more in-depth.
The group of interns were led to a dedicated practice room.
Associate Professor Fujita urged everyone to put on shoe covers and sterile gloves, and each person was assigned a workstation and specific positions according to their groups.
The practice room at the University of Tokyo's Faculty of Medicine is exceptionally large; more than thirty people can only occupy a quarter of the space.
The area is divided into three main sections. Lin Ze and his group are in the first section. In front of them is a sterile operating table, and all the suturing instruments are placed on a tray next to it. A silicone model is fixed on the operating table, which looks like a piece of skin, but with a slight color difference.
There's a light on top, so it doesn't need to be turned on during the day.
The cleanliness and tidiness of the workbench was Lin Ze's first impression.
When he saw the tweezers and special surgical needles and sutures, he almost couldn't resist the urge to look at them. He couldn't help but feel a strong urge to see them, but he managed to control himself from looking.
Without the permission of teachers or elders, touching or moving things without authorization will most likely result in criticism.
The equipment in the remaining two large areas was incomprehensible, especially the numerous white cylindrical plastic items on the central control panel, which were presumably for protecting equipment. No one had ever seen anything like it before and couldn't figure out what they were for.
It's worth mentioning that none of the interns had been to this practice room before. They had been to another teaching building, a very basic one, where bananas were placed on the worktable.
"I suppose you've learned it, but by now you've probably forgotten most of it."
At the very front of the stage, Professor Fujita adjusted his glasses, signaling for everyone to be quiet.
He said, "However, it's best to forget about it, because you didn't learn it well before. Start practicing the correct posture from the beginning, and you'll achieve twice the result with half the effort in practicing suturing techniques."
Sunlight streamed in from outside, making the practice room bright and clean, and everyone listened attentively to Professor Fujita's speech.
"Let me demonstrate first and explain the key points of the movements. The suturing technique must be standardized. First is quality, and second is speed. You all understand quality, but I need to explain speed. Simple suturing needs to be quick to reduce the patient's trauma time. Otherwise, there are many patients who don't receive anesthesia. If you sew for half an hour, wouldn't they be in excruciating pain?"
Upon hearing this, everyone chuckled.
"Furthermore, we need to guard against germs. Another point to note is for large-scale surgeries, where time is precious. We should try to compress the surgery time as much as possible, because time is life."
Associate Professor Fujita did not put on sterile gloves. He scanned the room, his gaze sweeping over the faces of the interns before finally settling on Lin Ze.
"Lin Ze," he beckoned gently, "come up and demonstrate."
It wasn't that Fujita was being unfair or deliberately giving the professor's student face; it was just that he only remembered Lin Ze.
All eyes immediately turned to them.
Lin Ze's seat was at the back of the practice room, some distance from the podium, and he hadn't expected to be approached like that.
Without making excuses, he walked directly down the corridor.
Waiting to stand on the podium.
Associate Professor Fujita stood to his side, his hands behind his back, while Lin Ze was facing the control panel in front of him.
On the large screen behind him, the sterile gloves Lin Ze was wearing instantly appeared. Undoubtedly, every action he took would be magnified for his classmates to see.
Lin Ze's gaze remained calm.
He had never tried the suturing technique provided by the system before, and he was a little unsettled. Surgery requires a steady hand and a calm mind, and even though he was usually calm, he needed to adjust his breathing at this moment.
Forget it, why think so much?
Lin Ze took a few deep breaths, and his mind calmed down again.
Mistakes are normal; someone who can't accept failure can't keep improving.
"Stand still and prepare for surgery," Associate Professor Fujita said calmly.
Lin Ze remained noncommittal, then straightened his back in an instant.
“Selection of suture instruments”.
Lin Ze raised his hand as soon as Associate Professor Fujita finished speaking.
but.
His mind suddenly went blank.
Even if his teacher had taught him about it a long time ago, Lin Ze couldn't recall the specific procedures immediately. His head was filled with too much theoretical knowledge, but this wasn't an exam.
He remained motionless for several seconds, only raising his hand.
Under the podium.
Miyagi Suzuo's gaze was fixed on Lin Ze's face. She unconsciously clenched her sleeve, her eyes filled with worry.
Clearly, she was even more nervous than Lin Ze.
“Who remembers that?” Otomo Shoji said to Hiramomi Onoko, who was standing next to him.
“I forgot too,” Hiramomi Onoko answered subconsciously, but then she frowned: “Don’t talk, keep quiet.”
Among the group, almost no one could remember.
But on the podium, Lin Ze suddenly moved, quickly grabbing the equipment next to him.
He remembered.
After a brief moment of confusion, the knowledge I had learned quickly flooded my mind.
The teacher's voice from the classroom seems to echo in my ears. "Needle holder, tissue forceps, suture scissors, suture needle, suture thread, sterile gauze, saline solution, and alcohol swabs."
He named each piece of equipment one by one.
Associate Professor Fujita was about to make a warning, but before he could even speak, he saw Lin Ze's actions, and then one by one the instruments were placed in the sterile tray.
A hint of relief appeared on his weathered face.
Then, Lin Ze accurately recited the name of the instrument.
"disinfect."
The people below the podium hadn't even caught their breath yet.
Especially for Miyagi Suzuo, her heart had just been hanging in suspense, and she had been holding her breath the whole time. Just as a hint of excitement appeared in her eyes because of Lin Ze's reaction, Professor Fujita's instructions came the next second.
Without hesitation, Lin Ze quickly disinfected the tray.
“At all times, wear sterile equipment,” Fujita explained to everyone. “Your hands and forearms must never come into contact with medical instruments.”
After explaining the important points.
As soon as he turned his head, Lin Ze was already in the suturing position.
To Professor Fujita's surprise, Lin Ze's movements were actually quite standard.
He held the needle in his right hand, gripping the lower half of the needle holder with four fingers, leaving some space in his palm, and using his thumb and other fingers to clamp the suture needle tightly.
Make sure to hold the needle firmly at the junction of the posterior one-third of the suture needle position.
Holding tissue forceps in the left hand, the arm rests on the operating table to avoid dangling in the air and to keep it perfectly within the sterile area.
Associate Professor Fujita paused for a moment, a hint of doubt in his eyes, and circled Lin Ze with his hands behind his back.
He couldn't find a single fault with it.
“Very standard,” Associate Professor Fujita nodded, then turned to look at the audience: “Remember, pay close attention to Lin Ze’s movements: four fingers down, palms with space.”
He raised his hand and pointed to his palm.
"If there is no space in the palm, the entire grip on the needle will be stiff and difficult to adjust. The four-finger grip is to prevent slipping."
As everyone watched Lin Ze's posture, they all became focused and gradually shifted from feeling the subtle pressure that Associate Professor Fujita was putting on Lin Ze to listening attentively to the lecture.
Some people have already tried to hold the needle and adjust their posture.
For example, Hiramika Onoko and Miyagi have already started learning it.
"You must hold the needle close to the needle tail, but not beyond the needle tail hole. Otherwise, if you make a slight mistake in your force application, the needle will bend. Do you understand?"
Lin Ze listened to his explanation as well, but at this moment, a slight ripple was stirring in his heart.
With almost no hindrance, he grasped the most suitable way to hold the needle when he held the needle holder.
It was so smooth that he felt like he had muscle memory for it.
Clearly, this is the function of the system.
After all, achieving the mastery level requires 30000 practice sessions.
"Lin Ze, classmate."
"I'm here, teacher."
"You can start sewing now. Don't be nervous. Your needle-holding posture is very standard. It's okay if you sew a little slower. I will correct your mistakes and demonstrate for everyone."
"Understood." Lin Ze nodded.
He took a deep breath.
The model on the control panel was exceptionally clear in his field of vision. He focused his gaze vertically and held the needle holder, aiming it at the wound.
Everyone watched in disbelief as the needle tip approached the model's tissue on the large screen.
next moment.
Lin Ze used his elbow to find the most suitable fulcrum, and with the skillful movement of his wrist, his entire arm moved horizontally on the control panel fulcrum.
As the needle holder is inserted, the primary hand moves, and the secondary hand, holding tissue forceps, follows suit.
He fixed the needle in place, inserted it at a constant speed, fine-tuned the body angle, and sewed the sutures back and forth with incredible speed. Throughout the entire process, his hands did not tremble in the slightest.
Lin Ze's eyes were sharp; he had no other thoughts in mind. The moment the needle holder fell, he entered a wonderful state of flow.
All I could see was the wound; I heard no other sounds, and my vision was even clearer than usual.
He could even feel the sutures sliding inside the model's tissue, again and again.
After a while.
After finishing the final knotting, he instinctively picked up the scissors and snipped them.
Looking at the stitched result that resembled a work of art, he felt a surge of joy from within.
But what Lin Ze didn't notice was...
The entire practice room was completely silent, so quiet you could hear a pin drop.
For the first time, Professor Fujita's weathered face showed astonishment as he stared blankly at the model on the control panel.
