Rebirth 1977 Great Era
Chapter 842 A Chance Encounter: Yu Yunxiu's Accomplice
Chapter 842 A Chance Encounter: Yu Yunxiu's Accomplice (Two Chapters Combined)
Fang Yan's attitude made Zhou Zhongping and Lu Xiuwen secretly breathe a sigh of relief.
After all, they were afraid that Fang Yan would hold a grudge. They had figured it out: if Fang Yan were to actually take action, he would never do something like Gu Mingyuan, which involved some harmless provocation.
He went straight for the heavy hand.
Don't be fooled by Gu Mingyuan's tough talk just now. Unless he suddenly has a medical breakthrough that surpasses Fang Yan's and can treat a patient, all his actions were just bluffing.
Based on their long time together, they knew that Gu Mingyuan was not that kind of person.
Gu Mingyuan does not have that ability.
All those things he just said were just him trying to maintain his last shred of dignity.
In layman's terms, it's like "a stubborn duck that can't speak properly."
Previously, Gu Mingyuan had caused trouble for Fang Yan and even dragged them into it. Now that Gu Mingyuan has been dealt with, they are all a little afraid that Fang Yan will hold them accountable.
I didn't have a deep enough understanding of dialects from newspapers before. Later, after learning about dialects through other channels, I found that they were far more terrifying than I had imagined.
Their background and the extent of their experiences are far beyond what people their age could possibly possess.
Humans are strange like that; they often harbor great hostility towards someone they can catch up to with effort.
But what if you encounter someone you can't catch up to no matter how hard you try, and the gap keeps widening?
At this point, everyone will become polite to this person again.
I have absolutely no desire to compete with him.
They even felt lingering fear when they recalled their previous actions.
Fortunately, Fang Yan isn't like Gu Mingyuan in terms of personality.
As soon as his own people showed him goodwill, Fang Yan immediately chimed in, without any airs.
This is exactly the result they wanted.
"Doctor, can I see a doctor here?" A voice came from the doorway.
The crowd looked in the direction of the sound and found a man in his fifties.
"The doctor just went out and might not be back for a while," Fang Yan said to the man at the door. He had no intention of doing anything himself in Chen Keji's consultation room.
Seeing that Fang Yan didn't respond, Yang Jingxiang understood and greeted the newcomer:
"Please come in and have a seat. He'll be back in a little while."
He didn't mention asking Fang Yan to help with the treatment.
The others were also shrewd, so of course no one brought it up.
But then the man said:
"Doctor...can you treat this? The situation might not be good."
Song Jianzhong looked at the man with some confusion:
You seem perfectly fine. Is something bothering you?
The man explained:
"It wasn't me, it was my dad."
"Your father?" Yang Jingxiang glanced towards the door.
No one was seen.
Then his gaze fell back on the man. If he was the man's father, he would be at least seventy or eighty years old.
The man nodded and said:
"Yes, it's placed in the hallway outside."
As he spoke, the middle-aged man called out to the outside, and soon an elderly man, who was trembling, was pushed in by a wheelchair.
“Brother Fang, this…” Yang Jingxiang turned his gaze to Fang Yan.
The only person here who is qualified to treat patients without angering Chen Keji is Fang Yan.
First, he was originally a doctor at Peking Union Medical College Hospital.
Secondly, Chen Keji had previously brought up the matter of using dialects to treat illnesses.
Fang Yan said to Zhou Zhongping and Lu Xiuwen, who were also looking at him:
"Director Chen is with your homeroom teacher. Can someone go and call him?"
"Me!" Lu Xiuwen was the first to react and said to Fang Yan.
Fang Yan nodded, and then said:
"Then go and call Director Chen back. I'll help you determine the patient's condition first."
Lu Xiuwen responded and then ran out.
Fang Yan turned to the old man in the wheelchair and asked:
"Old comrade, what's wrong with you?"
The old man spoke in a hoarse voice to the dialect:
"Urine!"
"Huh?" Fang Yan was taken aback. He had initially thought the old man's appearance indicated a lung problem.
Then he pointed to his lower body and said he was urinating.
“Let me explain,” the patient’s son said, taking over the conversation.
He spoke in dialect:
“My dad suffers from emphysema. A while ago, he suddenly developed a fever and said that he felt a tightness and stuffiness in his chest, as if something was blocking it. He also felt the same way in his abdomen. It was full and there was a strong feeling of pressure. He couldn’t touch it because it would hurt.”
"He hasn't had a bowel movement for a week, and he can't urinate at all. We felt the situation was very critical, so we took him to the emergency room of a nearby hospital for treatment. The doctor there diagnosed my father with..."
He paused, took out the examination report from his pocket, and pointed to it:
"So this condition is called... benign prostatic hyperplasia and urinary retention in the elderly."
Fang Yan nodded. Old Xie from the military compound had already discussed this matter before.
He went to the table, quickly wrote down the medical record, and then said:
"Okay, please continue."
The patient's son said:
"They said at the time that a catheter had to be inserted for urinary incontinence."
"After it was inserted, my dad was in a lot of pain, and he couldn't urinate at all because of the pain. In the end, we had to pull the catheter out."
"After all this back and forth, he still couldn't urinate at all. My dad was groaning and screaming in the ward, he was in so much pain."
"Moreover, at this time my father was unable to eat and was very weak. Generally speaking, he would need an IV drip, but the hospital did not dare to give him an IV drip, fearing that it would increase the pressure on his urethra."
"After spending two days in the emergency room there, we didn't get any treatment. In fact, our condition got worse. Our relatives suggested that we come and see a traditional Chinese medicine doctor. So we were discharged and came here to find a traditional Chinese medicine doctor to help us see if there was any other treatment option."
Upon hearing this, Fang Yan wrote down the medical case while analyzing it.
For the elderly, the condition of "lower yin" (lower yin) often refers to a deficiency of kidney essence, in which case their yin blood is dry and their body fluids are lacking.
In addition, this patient has a history of emphysema, and his upper lung qi has always been weak, making it unable to properly perform functions such as regulating fluid metabolism and qi circulation.
Traditional Chinese medicine theory emphasizes that "essence and blood share the same origin," meaning that essence and blood nourish and transform each other. When both essence and blood are deficient, and there is also a deficiency of qi, the circulation of qi and blood in the body becomes sluggish, resulting in symptoms such as difficulty in urination and defecation. Often, the condition will develop very rapidly.
Treatment should focus on nourishing lung qi and replenishing body fluids, addressing the root cause by treating the upper body.
However, his condition is very urgent, and there is definitely no time to nourish his lung qi and replenish his body fluids.
The problem that needs to be solved is that he can't urinate or defecate.
"Doctor, I feel so uncomfortable," the old man said to Fang Yan.
Fang Yan heard his words, glanced outside, and then said:
"I'll examine you right away, prescribe some medicine, and you'll get better soon."
"Open your mouth and let me see your tongue. Extend your hand so I can take your pulse."
Fang Yan decided that if Lao Chen didn't come back right now, he would take over for the time being.
It's impossible for me to cure him myself; I have to wait for him to come back.
Since the patient has already been admitted, there's no turning back now.
Of course, he also believed that Chen Keji would not blame him.
After the examination, Fang Yan found that the patient's pulse was deep and hesitant, and his tongue was red with little coating.
A red tongue with little coating and a deep, hesitant pulse indicate a deficiency of yin fluids and internal heat. The hesitant pulse also suggests poor blood circulation. Combined with constipation and urinary retention, this indicates qi stagnation, a history of emphysema, dysfunction of the upper jiao qi mechanism, and decline in the kidney's function of controlling urination and defecation.
In summary, the patient suffers from yin deficiency with excessive fire, qi stagnation and blood stasis, and deficiency of both lung and kidney.
"Biao Shi" refers to constipation, which is an acute condition.
Benxu: This refers to deficiency of lung and kidney yin combined with qi deficiency, which is a chronic underlying disease.
Just as I thought, a quick fix addresses the symptoms, while a slower approach addresses the root cause.
First, clear the bowels and bladder, then use a strong purgative to relieve the heat and stagnation, combined with methods to nourish yin and moisten dryness to resolve the obstruction.
After addressing the root cause, once bowel movements and urination become regular, focus on tonifying Qi and nourishing Yin, as well as replenishing the lungs and kidneys.
"Is the doctor better yet? Can he prescribe the medicine?" the old man asked Fang Yan anxiously.
Fang Yan glanced outside again, then nodded and said:
"Alright, you can prescribe the medicine now."
Then Fang Yan picked up the prescription sheet, laid out some review paper, and started writing. To explain his reasoning for the prescription, Fang Yan even labeled each of the herbs:
Scrophularia ningpoensis 30g (nourishes yin and reduces fire, promotes the upward flow of kidney water)
Ophiopogon japonicus 24g (moistens the lungs, promotes fluid production, and increases hydration)
24g of raw Rehmannia root (cools the blood, nourishes Yin, moistens the intestines, and promotes bowel movement)
Rhubarb 9g (added later) (clears heat, promotes bowel movement, eliminates waste and promotes metabolism)
6g of Glauber's salt (taken with water) (to soften and disperse nodules, assisting rhubarb in its purgative effect)
15g of North American ginseng (nourishes lung yin and benefits stomach fluids)
12g of Polygonatum odoratum (nourishes Yin and moistens dryness, and also protects stomach Qi)
Platycodon grandiflorus 9g (to promote lung function, lift the pot lid)
12g of peach kernels (promotes blood circulation, removes blood stasis, moistens the intestines and relieves constipation)
Angelica sinensis (tail) 9g (invigorates blood circulation, nourishes blood and moistens dryness)
15g of hemp seeds (for promoting bowel movements without harming the body's vital energy)
12g of Prunus japonica seed (promotes urination and bowel movement, guides pathogens downward)
Plantain seed 15g (wrapped and decocted) (promotes urination and draws heat downward)
Alisma plantago-aquatica 12g (promotes diuresis and eliminates dampness, relieves constipation and promotes bowel movement)
Raw Atractylodes macrocephala 15g (strengthens the spleen and replenishes qi, prevents damage to the body's vital energy)
Licorice 6g (to harmonize the effects of other herbs, relieve spasms, and protect the stomach)
It also includes instructions on how to decoct and take the medicine:
Add rhubarb last, and decoct for 15 minutes before adding rhubarb. Take the medicine with Glauber's salt, and dissolve the Glauber's salt in the hot decoction to enhance the effect of softening and dispersing nodules. The first dose is divided into 3 doses, with an interval of 2 hours between each dose, and the dosage is adjusted according to the bowel movement.
This was mainly written for the people at the pharmacy.
Given the old man's current condition, it's impossible to send him back. Fang Yan can't arrange for him to be hospitalized, so they have to wait here and see what Chen Keji decides later.
After finishing writing, Fang Yan looked at the crowd behind him and asked:
"Who will help collect the medicine? And while they're at it, let's decoct it too."
“I’ll go! I know this place pretty well.” Yang Jingxiang volunteered.
Fang Yan handed him the prescription directly.
Then Fang Yan took out the silver needles and said to the old patient:
"Sir, could you please lift up your clothes and trouser legs so I can start the needlework?"
As soon as he said that, the old man didn't even have to lift a finger; his son and daughter immediately helped him lift up his clothes and trouser legs.
"Brother Fang, need help with moxibustion?" Zhang Yanchang asked Fang Yan.
He said in dialect:
"No need, I'll just insert the needles, not do moxibustion."
After saying that, he began to insert needles into the old man's Guanyuan, Qihai, and Sanyinjiao points on both legs.
They all use the balanced tonification and sedation technique of the Three Talents Needle Method.
Not long after the acupuncture was administered, the old man addressed the crowd:
"I can feel it! I can feel it!" his son exclaimed.
"Big or small?"
"If you need to pee, it's a small one," the old man replied.
After saying that, he immediately said:
"I really feel it coming! Quick! Quick! Push me to the toilet!"
Soon the old man and his family pushed him to the toilet.
Before leaving, the patient's family gave a thumbs-up to the dialect.
At Capital Medical University, there are still Zhou Zhongping and another student left.
Upon witnessing this scene, the student exclaimed in admiration:
"Amazing!"
"I haven't even taken the medicine yet, and I'm already urinating!"
After speaking, he looked at Fang Yan with admiration in his eyes.
Zhou Zhongping said:
“He’s not just a student, he’s also the director of the Department of Traditional Chinese Medicine at Peking Union Medical College Hospital.”
Upon hearing this, the former nodded repeatedly:
"Oh, right, right..."
Zhou Zhongping noticed that Fang Yan was looking at him, so he quickly cupped his hands in greeting:
"As expected of Fang Yan, he recovered so quickly."
Fang Yan shook his head and said:
"It's not a cure, it just got him to urinate first."
After saying this, he held up the medical record and said:
"This patient has always had weak lung qi, and the lungs cannot properly perform their function of 'governing and regulating the body's physiological functions.' As a result, the downward flow of water in the body is obstructed, leading to urinary retention, which is what we in traditional Chinese medicine call 'urinary retention.'"
"In addition, if the lungs cannot perform their function of 'cleaning up and removing foreign objects and keeping clean,' then the large intestine will lose the moisture from the body fluids distributed by the lungs, leading to constipation. Therefore, the patient's body is in a state of damage to both qi and body fluids, ultimately resulting in a situation where urination and defecation are not smooth."
"The patient's emphysema has led to impaired lung function and obstructed water metabolism. Acupuncture at Qihai and Guanyuan points can stimulate the qi transformation in the lower jiao, like opening a kettle lid to promote the downward flow of fluids. The Nan Jing states: 'The Sanjiao is the pathway for water and grain, and the beginning and end of qi.' The Zhen Jiu Da Cheng states: 'Guanyuan is the main point for treating urinary retention and painful urination, while Sanyinjiao treats urinary difficulty and weakness in the legs.' Acupuncture at Sanyinjiao can regulate the metabolism of fluids in the lower jiao and restore the function of the Sanjiao as the organ of drainage. It is normal for him to have this reaction after I inserted the needles."
At this point, he added the patient's reaction to the medical record.
These are all basic skills, but not everyone can use them so smoothly.
Fang Yan still remembers when they first met, Gu Mingyuan introduced Zhou Zhongping, saying that he had created Shengjiang San to cure intractable asthma, and that his master was the former director of the Chinese Pharmacy Bureau, and that Zhou Zhongping himself was also proficient in processing medicinal materials.
This person must have some unique insights into lung diseases and the use of medicinal herbs.
Fang Yan said to him:
"Actually, this disease could be treated without any problems by anyone. The reason it has become so serious is mainly due to problems with the procedures performed by the Western medicine practitioners, coupled with the family's inability to make a judgment, which led to the old man having a catheter inserted and the delay until now. In fact, if they had come earlier, it wouldn't have been so difficult."
In conclusion, Fang Yan gave everyone a hand first.
What I mean is that my method is not a sophisticated one, and anyone can do it just as well.
Zhou Zhongping knew that Fang Yan was just being polite, so he said to Fang Yan, "..."
"Fang Yan, may I take a look at your prescription?"
The prescription was recommended in dialect:
"Please."
After looking at it, Zhou Zhongping said:
"Is it a modification of Zengye Chengqi Decoction combined with Wuling Powder?"
"Mm." Fang Yan nodded.
Zhou Zhongping examined it carefully and then began to analyze it:
"The combination of Scrophularia, Ophiopogon japonicus, and Rehmannia glutinosa, with each herb's dosage increased by 50% compared to the original formula, is likely a modification for severe yin deficiency in the elderly. The ratio of rhubarb to Glauber's salt is 3:2, ensuring both effective bowel movement and avoiding excessive depletion of body fluids. Platycodon grandiflorus is combined with Adenophora stricta to... promote lung qi to clear the water passages and improve qi stagnation caused by emphysema!"
"Peach kernels combined with angelica root tails form a blood-activating and bowel-moistening combination, improving intestinal blood circulation. Hemp seeds combined with apricot kernels enhance intestinal lubrication, targeting senile dysentery."
"Furthermore, the combination of raw Atractylodes macrocephala and licorice here protects the spleen and stomach in a strong purgative, preventing the risk of deficiency. It's quite impressive that this combination was devised in such a short time!"
Zhou Zhongping looked at the prescription over and over again, and it seemed to be the result of careful thought and precise calculation.
It's completely unlike something that someone in a dialect could come up with so quickly.
This is a modified formula, not the original classic formula. Only someone like Zhou Zhongping, who has a special understanding of medicinal materials, would know the value of this formula.
"What are you shushing about?" The kid next to him didn't pay attention to the main point at all, and instead asked about the last sentence, "the Ring of Illusion."
Zhou Zhongping said to his classmates:
The "Treatise on the Five Constant Principles of Traditional Chinese Medicine" in the Suwen states, "Do not let excess become more excess, nor deficiency become more deficiency, lest it lead to premature death and disaster." This emphasizes that excess should not be made more excess, nor deficiency more deficiency, so as not to bring misfortune to the patient. This theory has been constantly emphasized and followed in the clinical practice of traditional Chinese medicine, and it can actually be considered one of the basic principles of traditional Chinese medicine treatment.
“Gu Mingyuan hadn’t paid attention to this before. Fang Yan said that this was the first question he asked.”
After he finished speaking, the former finally realized what was going on.
Zhou Zhongping then asked Fang Yan:
"Fang Yan, did you come up with this combination on the spot?"
"Yes," Fang Yan nodded.
Just then, footsteps sounded outside the door, and soon Lu Xiuwen returned with Chen Keji.
"Huh? Didn't you say there was a patient? Where is the patient?" Chen Keji asked.
The dialect responded:
"I just got an acupuncture needle and I'm going to the toilet now."
At this opportune moment, Zhou Zhongping handed the detailed medical record he had just received to Chen Keji.
"Director Chen, this is the medical record that Fang Yan just wrote."
Chen Keji took the medical record and glanced at it, then found that it was written in great detail.
The cause and effect have all been written out.
He watched as if he had seen it with his own eyes, and he nodded repeatedly in agreement with the idea of using dialects to solve the problem.
Just then, a sound came from the doorway.
"Doctor, my dad finally peed!"
When everyone turned around, the patient had already been brought back in a wheelchair by his children.
The old man chuckled as he spoke in dialect:
"Thank you so much, doctor! I've finally been able to pee after being constipated for so many days! I wish I had come to your traditional Chinese medicine clinic sooner. The hospital over there gave me so much trouble, and it didn't help at all..."
After saying this, he addressed his children:
"I always said that we Chinese should use traditional Chinese medicine to treat illnesses, right?"
His children also gave awkward smiles and then nodded repeatedly.
After all, it was they who took the old man to a Western doctor to have a catheter inserted.
After much trouble, it still didn't work.
At this moment, the old man said to Fang Yan:
"If you're going to go through all this trouble, please help me unload the stuff in my belly too! Where do you want the injection, should I lift my shirt or my pants?"
Fang Yan glanced at Chen Keji, who nodded at him, indicating that he should handle the matter.
So Fang Yan said to the old man:
"You'll have to wait a bit, the medicine is being decocted."
"Oh..." The old man suddenly realized that there was still a dose of medicine left.
Fang Yan then asked Chen Keji:
"Director Chen, could you please admit me to the hospital?"
Chen Keji was taken aback, then nodded:
"Okay! I'll write a note, old comrade, you can have your family help you with the hospital admission."
The old man did not agree to Chen Keji's request. Instead, he looked at Fang Yan, still believing more in the young man who could make him urinate with three injections.
Fang Yan said to the old man:
"Old comrade, your illness still needs treatment; you'll need to stay in the hospital for a few days."
"Oh, okay!" The old man nodded and agreed to be admitted to the hospital.
Fang Yan added:
"Also, while hospitalized, avoid eating mutton, leeks, and other warming and drying foods. It's best to eat porridge made with things like white fungus and lily bulbs."
"Okay, sure!" The patient and his family agreed.
Then Chen Keji finished writing the order.
They went to complete the hospital admission procedures.
Fang Yan also asked Zhang Yanchang to go to the pharmacy and tell them that the person had been admitted to the hospital, and to bring Yang Jingxiang back with him.
After everyone left, Chen Keji spoke to Fang Yan:
"The prescription you wrote is an innovative combination based on classic formulas. It not only follows the treatment principle of the six viscera being unobstructed, but also takes into account the physical characteristics of elderly patients with weak vital energy. Through the precise balance of nourishing yin and moistening dryness and purging heat accumulation, it achieves the treatment goal of 'attacking the pathogen without harming the body's vital energy, moistening dryness and promoting blood circulation.' You have mastered the essence of TCM syndrome differentiation and treatment."
Fang Yan said to Chen Keji:
"You flatter me. We still need to closely monitor the patient's response and adjust the ratio of attack to supplementation in a timely manner to ensure the treatment is safe and effective."
"The rest of the treatment will depend on Director Chen."
Fang Yan couldn't stay here to look after the old man in the hospital, so everything had to be handed over to Chen Keji.
Chen Keji nodded and handed the medical record book in his hand to Fang Yan:
"Once you've finished writing the medical record, I'll arrange the follow-up treatment."
"Okay." Fang Yan took it and quickly wrote down all the previous details.
Chen Keji discovered that Fang Yan's medical records were incredibly detailed. If Fang Yan were still an unknown student, he would have wanted to hire him as an assistant.
Unfortunately, he is the director of the Department of Traditional Chinese Medicine at Peking Union Medical College Hospital, and when it comes down to it, I am still a notch below him in terms of dialect.
The head of the Traditional Chinese Medicine department over there is the only one of his kind.
Moreover, it is a favorite of the Ministry of Health, the Overseas Chinese Affairs Office of the State Council, and Peking Union Medical College Hospital.
It's nothing compared to what I've achieved.
"By the way, has Gu Mingyuan's matter been settled?" Fang Yan asked Chen Keji.
Chen Keji said:
"I told their teacher, but it might be difficult to persuade them to withdraw..."
"This kid has some connections."
"Is that so?" Fang Yan frowned slightly.
Chen Keji said:
"His father was a student of Wang Qizhang."
Fang Yan was taken aback and asked:
"Wang Qizhang? The Wang Qizhang that Yu Yunxiu was with?"
"Mm." Chen Keji nodded.
Fang Yan burst out laughing when he heard this.
Well, no wonder I don't like him and he doesn't like me either. There's something deeper going on.
Yu Yunxiu was a representative figure of the modern school of thought that advocated abolishing traditional Chinese medicine. Besides him, there were other like-minded people, including Wang Qizhang, Pang Jingzhou, and Fan Shouyuan...
The history of Wang Qizhang is also quite interesting.
