Chapter 1053 Conservative Treatment of Anal Carbuncles

Chapter 1053 Conservative Treatment of Anal Carbuncles (Bonus Chapter for Monthly Tickets)

This anal abscess, also known as perianal abscess, is divided into several stages.

The initial stage is also called the pus-forming stage.

It is a lump that suddenly appears around the anus, the pain gradually worsens, and the area becomes red, swollen, hot and painful, and feels like a hard lump.

At this time, you may experience a mild fever, fatigue, constipation, or dark yellow urine.

Next comes the second stage, also known as the suppuration stage.

At this point, the lump has usually been there for some time. The lump will grow larger, and the pain will become more intense, making it difficult to sit or lie down. The small lump may become a large lump or swell up completely. High fever will occur in that area, and other symptoms will also appear throughout the body, such as high fever, chills, dry mouth, bitter taste, and constipation.

At this point, the abscess will enter a bifurcation stage. It may break through the skin and drain the pus, or it may spread into the body and rot a large area under the skin.

If you're unlucky, it can even cause necrotizing fasciitis.

The second stage is also the most common stage in hospitals. Most patients come to the hospital for help after trying antibiotics or receiving intravenous infusions at small clinics without success.

The third stage is the post-collapse stage.

If the abscess doesn't fester inside the body but instead ruptures and drains through the skin, the pain will quickly subside, and the redness and swelling will also disappear rapidly. In a lucky few cases, nothing will happen at all.

However, most people will experience repeated infections and develop anal fistulas because the primary infection site is not treated. This is characterized by repeated discharge of pus, pain, and itching from the same spot.

Regardless of the stage, in my previous life, most hospitals recommended surgery after receiving these people.

They will tell the patient that surgery is the only option and that conservative treatment is not possible.

There are several main factors. The first is the fear that the pus in the patient will develop too quickly and spread into the body, forming necrotizing fasciitis.

Arranging surgery directly is definitely the safest option.

Secondly, there's the consideration of efficiency. Pure Chinese medicine treatment often takes one or even two months, while surgery is much more efficient.

Third, of course, there are economic considerations. Surgery is definitely more profitable than pure traditional Chinese medicine treatment, considering all the examinations, surgery, nursing care, dressing changes, etc.

However, a small number of experienced doctors will still recommend traditional Chinese medicine treatment first. Firstly, it can avoid surgery and pain for the patient. Secondly, it can not affect the patient's normal work and life. Drinking a mouthful of medicine every day and soaking in it at night is much better than having surgery on the buttocks, lying in bed for several days, and having to go to the hospital every day for more than 20 days to change the dressing.

Moreover, after surgery, it is necessary to take medication and clean the wound.

Hearing the dialect, He Puren asked:
"Do I still need to wash?"

Fang Yan shook his head and said:
"It needs to be soaked; just washing it isn't enough."

After speaking in dialect, he asked He Puren:
"Where is Lao Zeng right now?"

He Puren glanced at his watch and said:
"They should have gotten the medicine from the hospital and gone home to brew it."

He said in dialect:

“Okay, I’ll go get his medicine and deliver it to his house later.”

He Puren probably never expected that Fang Yan would even need to deliver medicine to his door. He was truly an excellent leader.

He nodded.

Fang Yan said to him:
"Alright, you can stay here and do your work. I'm going to get the medicine."

He stopped abruptly after speaking in his dialect, and pulled the box of the "Pandemic Needle" from his pocket:

"By the way, this needle is something I brought back from Hong Kong. It's called the 'Prairie Fire Needle,' and it's a kind of fire needle I created myself. See if it can help your fire needle research."

Upon opening the box, He Puren was slightly taken aback by the design of the Prairie Needle.

Sensing keenly that the needle was unusual, he nodded:
"That's interesting... I'll take a look."

Fang Yan said to him:
"Don't break it, it was a gift from someone."

He Puren nodded:
"Don't worry, I know what's appropriate."

Fang Yan then said goodbye to He Puren and went to the acupuncture hospital next door.

I first came to this place when I was looking for Professor Cheng. At that time, Professor Wang Yuchuan was here leading fifth-year students on their internship.

That is, Xie Chunrong, Zhao Qingkai, Chen Wenwei, and the others.

Before arriving at the hospital's traditional Chinese medicine pharmacy, Fang Yan even took out paper and pen to write down the prescription.

This is a prescription he secretly learned from the old doctor.

Formula composition:

Add 1500-2000ml of water to the herbs, bring to a boil over high heat, then simmer over low heat for 15-20 minutes. Filter and extract the juice.

Once the liquid has cooled to 40-45℃ (not too hot to scald the skin), pour it into a basin. First, use the steam to fumigate the skin, then sitz bath for 15-20 minutes each time, 1-2 times a day.

Of course, as the old Chinese medicine doctor said, if you have the time, it's best to do it three times a day, in the morning, noon, and evening.

However, this prescription is rarely used because many people come to us after the disease has progressed to the second stage.

In more severe cases, the buttocks may swell up.

At this point, we need to use another method.

The prescription is called "Mangxiao Kushen Decoction". Its components are:

芒硝(后下)15g、苦参20g、黄柏15g、川芎12g、赤芍15g、当归尾12g、乳香 10g、没药10g。

Except for Glauber's salt, decoct the other medicinal materials for 15 minutes first, then add Glauber's salt and boil for 2-3 minutes, then filter and take the juice.

After the medicinal solution reaches a suitable temperature, take a sitz bath 1-2 times daily. Local hot compresses can be used to enhance the therapeutic effect.

This prescription is actually similar to the internal medicine remedy, Xianfang Huoming Yin.

They all reduce swelling and pain, promote blood circulation and remove blood stasis, and relieve symptoms.

In short, as long as the redness and swelling don't develop rapidly during this period, and it's not like a small spot in the morning that swells up to half of the buttocks by the afternoon, this method can be used in conjunction with oral medication for treatment.

After all, few patients would want to undergo surgery right away.

Fang Yan took the prescription to the traditional Chinese medicine pharmacy, got the medicine, and immediately returned to the research institute.

We found Zeng Luquan's home.

Their family has already moved from the school area, and the conditions here are much better than at the school.

Fang Yan knocked on the door.

"Who is it!" Zeng Luquan's voice came from inside the room.

“Me!” Fang Yan replied.

Then came the pattering sound of slippers running, and soon the door was opened.

"Hey! You're back!" Zeng Luquan said with a look of surprise when he saw Fang Yan outside the door.

Fang Yan held up the ten doses of the prescribed medicine and said to him:
"Well, I heard from Lao He that you were sick, so I went and got you some external sitz bath medicine."

Zeng Luquan was stunned when he saw the medicine packet in Fang Yan's hand.

They probably didn't expect that dialects would be used so widely.

"Come inside quickly," Zeng Luquan invited Fang Yan.

As she spoke, she took the medicine from Fang Yan's hand.

As soon as Fang Yan entered the door, he smelled a strong medicinal odor wafting from the direction of the kitchen.

Fang Yan pointed behind Zeng Luquan:

"So, how's the situation now?"

This question might be a bit embarrassing for an ordinary person, but since we're all practitioners of Traditional Chinese Medicine, we didn't feel it at all, Zeng Luquan said:

"A hard lump the size of a fingernail, which has not yet developed."

Fang Yan nodded and said:

"Then the medicine I prescribed should be suitable."

After saying that, he tapped the table again and said:

"Come on, let me take your pulse again."

P.S.: I still owe you 47000 words after finishing this chapter.
